Health Information & Forms

School health at St. Patrick’s is being provided by and under the supervision of a Registered Nurse, with the assistance of trained school office staff and volunteers, for the health and well-being of our school students. To do this, St. Patrick’s follows laws and guidelines set forth by the Nebraska Department of Health and Human Services.  

 

The State of Nebraska has set forth requirements for physical examinations, visual evaluations, and immunizations. Please refer to Physical Examination/Visual Evaluation/Immunization Requirements for details related to these requirements for students entering school in Nebraska.  We have a Health Examination Card available in the school office for completion at the time of a physical exam.  The Vision Evaluation Form and Immunization Information Form may be obtained at the links below. See the Summary of the School Immunization Rules and Regulations link for complete list of immunization requirements. St. Patrick’s requires parents/guardians to update the school at least annually with information related to their child’s health.  This is the Health Update Form link is below.  Additional updates are required throughout the school year, if your child has a significant change in their health.  If your child has a health condition requiring special consideration or action at school, additional forms may be sent home to you for completion.  This information will provide school staff with accurate information regarding your child’s needs.  If your child will need any medication during school hours, you must complete the appropriate forms giving your permission for it to be taken at school.

General screens for vision, hearing, dental, height, weight, blood pressure, and scoliosis will be done throughout the school year.  Parents will be notified when screens will take place.  Parents/guardians will be notified of the results, after screening results have been review by the School Nurse and only if additional evaluation or follow-up is recommended.
